Terms of Service
SCOPE
The client is aware that coaching (including relationship coaching for singles, dating couples, engaged couples, married couples, sexual integrity coaching, etc.) is in no way to be construed to be a substitute for any form of therapy, counseling, medical, or legal advice. The client acknowledges that the provider/coach will not be liable legally or otherwise for any actions he or she may or may not undertake as a result of coaching sessions. There is no assumption of responsibility made, or given, and the client agrees not to hold Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships), or its providers/coaches, responsible or liable in any form or fashion for any actions taken by the client. The method and process by which suggestions or direction are presented in no manner whatsoever, written or verbal, constitutes an agreement or liability on the part of the provider/coach.
WARRANTIES
The client agrees that using any of the coaching services are entirely at your own risk. Coaching services are provided "as is," without warranty of any kind, either expressed or implied, including without limitation any warranty for information services, coaching, uninterrupted access, or products and services provided through or in connection with the service. The client may periodically be provided with links to other resources, information, services, or products which you may find beneficial. However, this does not constitute endorsement of such products, services, or resources. In no event shall Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships), or its providers/coaches be liable for any resulting incident or consequential damages.
ELECTRONIC COMMUNICATION
I understand that if we engage in any coaching over the Internet, I consent to having sensitive information transmitted electronically (Internet-based communication, such as email, or other Internet or Web-based services) and acknowledge the risks involved, and waive any rights against Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships) or their coaches/providers for damages arising from any transmission, mistakes or errors made in connection with any such transmission.
PROPRIETARY APPROACH
I understand that coaching methodology, including principles, concepts, coaching techniques and training, as well as assignments, email, attachments, and documents, are proprietary and I understand and agree that they may not be used for any purposes, other than this coaching relationship, without written consent from an officer of Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships). The principles learned in coaching may be implemented and used by me personally, but I may not speak, publish, or distribute publicly outside without written consent of an officer of Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships). I understand that information will be held as confidential, provided however that such information may be shared with Whittaker Institute for Success and Enrichment, LLC (DBA Center for Passionate Relationships) coaching professionals for consultation purposes, or as required by law.
SERVICE SELECTION
I further understand that each 50-minute coaching session will be $115, which is to be paid in advance of the coaching session. Payments are made via PayPal or credit card at least 48 hours prior to the coaching session. The scheduling and payment for sessions occurs through http://www.centerforpassionaterelationships.com/ . The coach / provider will be available to the client by e-mail and voicemail in between scheduled meetings as outlined by the coach.
POLICIES & PROCEDURES
The client will initiate all scheduled phone calls, and in cases in which other communication is used (such as Skype for example) the client will be provided with the link and access information for the session. The client will be punctual in contacting the coach for all scheduled sessions. Prior to each session, the client will determine the session agenda. The client will email the agenda to the coach, along with any necessary supplementary information, 24 hours prior to each session. The client will participate in the sessions and engage with a disposition ready to take relevant action and make use of feedback. In order to make the most of this working relationship, it’s important for both the coach and the client to freely express their thoughts, preferences, and needs; therefore, open dialog is greatly encouraged.
CANCELLATION POLICY
If the client needs to reschedule or cancel a scheduled session/meeting with the coach, it is the client’s responsibility to notify the coach 24 hours in advance of the scheduled call/meeting to reschedule or cancel. The coach reserves the right to bill the client for a missed meeting; however, the coach will attempt to reschedule the missed meeting if possible.
TERMINATION
The client or the coach may terminate this agreement at any time with verbal or written notice.
REFUND POLICY (Applies only to prepaid sessions)
All prepaid, unused sessions expire 3 months after initial payment. (Note: this refund policy this only applies in cases where the client has prepaid for multiple sessions).
ACKNOWLEDGEMENT
This agreement, in its entirety, reflects a complete understanding of the involved parties, and supersedes all prior written and oral representations. The form needs to be completed and submitted at least 24 hours prior to your first session. By typing your full name, email address (this should be the same email address where you will be receiving communications and coaching information), date of birth, and the current date, you acknowledge that you have read, agree to, and accept the terms of service described within this document. If you wish to retain a copy of this agreement, please print a copy for your convenience.
